- · Sessions are $89/week, billed weekly via the agreed payment method
- · Cancel with one week's written notice — no lock-in contracts

- · Student information is stored privately in Notion and not shared with third parties
- · Submitting this form constitutes agreement to the full Axis Tutoring Service Agreement
